What were the key signs and symptoms presented in the scenario that guided your choice of drug therapy?Key signs and symptoms were the patients presenting with a current issue. In one scenario, a 63 year old man with chronic stable angina felt angina during exertion. Nitroglycerin could be started in this case as it can be taken as needed for patients with stable angina.
What considerations did you consider regarding the patient’s medical history and potential drug interactions?Some considerations I made was the patient’s current history. For example, a 70 year old female with a recent NSTEMI should be started on Plavix to reduce the risk of another cardiac event.
How did you assess the potential side effects and manage the patient’s supportive care needs?Some of the scenarios presented with patients with side effects of current medications. In these scenarios, I looked for an alternative medication that fulfilled the same role, did not have any contraindications, and did not have the current issue the patient presented with as a side effect. For example, in one of the scenarios, a 65 year old with stable ischemic heart disease was suffering from erectile dysfunction while on metoprolol. Ranolazine was a suitable alternative as it fulfills similar roles, is a different medication class, and does not have erectile dysfunction as a side effect.
What clinical guidelines or reference(s) did you use during the activity?The clinical guidelines I referenced included UpToDate and researching the medication options on National Institutes of Health.
What did you like or dislike about this learning activity related to your understanding and application of pharmacologic principles?What I enjoyed about this learning activity is that selecting an answer required proper reasoning. It is important to understand the rationale of the correct answer because a correct answer can be right for the wrong reasons.

Then, we were given information regarding their vital signs, other comorbidities, and any contraindications for the choices provided. I used the Up To Date resource to verify the evidence-based recommendations and clinical guidelines to choose the appropriate therapy in each scenario.
What considerations did you consider regarding the patient’s medical history and potential drug interactions?I would consider the patient’s comorbidities and current medications when selecting the right drug to address the chief complaint. Also, any contraindications for the choices available.
How did you assess the potential side effects and manage the patient’s supportive care needs?To manage potential side effects, I researched each drug recommended. Then, I researched the potential side effects to determine if the drug would be both safe and effective for the patient. For supportive care, I offered recommendations such as following a heart-healthy diet low in saturated fats and rich in whole grains. I encouraged the patients to engage in light to moderate exercise for 30 minutes daily. For patients with chest pain, I educated them on the use of nitroglycerin and when to seek medical care. For the patient with heart failure, I educated the patient on sodium and fluid restriction and the use of daily weights to manage symptoms of fluid overload.
What clinical guidelines or reference(s) did you use during the activity?I used the Up To Date resource. I used it last week for assignments in NURS560, and I found it very helpful. It provides clinical practice guidelines for medical professionals. It’s easy to navigate and provides an abundance of education for the user as well as patient-friendly educational materials.
What did you like or dislike about this learning activity related to your understanding and application of pharmacologic principles?I enjoyed the activity. I find that I learn best when I can apply what I’m learning to real-life scenarios. The AI used was easy to navigate and provided helpful feedback for both correct and incorrect answers. I also liked that if I was wrong the first time, it allowed me the opportunity to correct myself using the rationale provided.
